1. Acknowledgment of Risks

I, the undersigned parent or legal guardian of the minor participant listed, understand that participation in fitness activities and the use of equipment and facilities at 4everBods Fitness Club involves inherent risks. These risks include, but are not limited to, physical injury, muscle strain, broken bones, cardiovascular incidents, and other potentially serious injuries.

I acknowledge that 4everBods Fitness Club has informed me about these risks and that I, on behalf of my minor child, voluntarily choose to participate in these activities and/or use the facilities with an understanding of the possible risks involved.

2. Assumption of Responsibility

I, the undersigned, on behalf of my minor child, voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by my minor child, or any loss or damage to property owned by my minor child or myself, as a result of my child’s participation in activities at 4everBods Fitness Club.

3. Release of Liability

I hereby release, waive, discharge, and hold harmless 4everBods Fitness Club, its employees, trainers, agents, representatives, affiliates, and all individuals involved with the club from any and all liability, claims, demands, or causes of action that may arise from my child’s participation in activities at the club or use of the facilities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. Medical Consent

In the event of an emergency or incident requiring medical attention for my child, I authorize 4everBods Fitness Club and its representatives to secure, at my expense, necessary medical treatment, including but not limited to first aid, transportation, and emergency medical services.

5. Rules and Regulations

I acknowledge that 4everBods Fitness Club has established rules and guidelines for the safety and well-being of all participants. I agree to ensure that my minor child abides by these rules. I understand that failure to follow these guidelines may result in the revocation of my child’s access to the facilities and activities, without any refund.

6. Indemnification

I agree to indemnify and hold harmless 4everBods Fitness Club from any loss, liability, damage, or costs they may incur due to my minor child’s participation in activities or use of the facilities, whether caused by negligence or otherwise.

7. Acknowledgment of Understanding

I have read this waiver and release and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily, intending it to be a complete and unconditional release of liability to the greatest extent allowed by law.
