Wellness Services Liability Waiver & Release

Wellness Services Covered

This waiver applies to participation in any wellness or recovery services offered by 4everBods Fitness Club, including but not limited to:

  • Infrared Sauna
  • Cold Plunge / Cold Water Immersion
  • Compression Therapy (e.g., pneumatic compression devices)
  • Massage Chair


Acknowledgment of Risk

I understand that the wellness services listed above involve inherent risks, including but not limited to dehydration, dizziness, fainting, muscle soreness, strain, changes in blood pressure or heart rate, aggravation of pre-existing conditions, burns, cold exposure injuries, or other physical injury.

I acknowledge that 4everBods Fitness Club does not provide medical advice, diagnosis, or treatment, and that these services are voluntary and intended for general wellness and recovery purposes only.


Medical Responsibility

I certify that:

  • I am physically and medically able to participate in these services.

  • I have consulted a physician if I have any medical conditions including (but not limited to) heart conditions, circulatory issues, high or low blood pressure, respiratory conditions, pregnancy, skin sensitivity, neurological conditions, or recent injury or surgery.

  • I will immediately discontinue use and notify staff if I experience discomfort, pain, dizziness, numbness, or any unusual symptoms.

Assumption of Risk

I voluntarily assume full responsibility for all risks, known and unknown, associated with participation in these wellness services.


Release & Waiver of Liability

In consideration for being permitted to participate in wellness services at 4everBods Fitness Club, I hereby release, waive, and discharge 4everBods Fitness Club, its owners, employees, contractors, and affiliates from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to my participation, including those caused by negligence, to the fullest extent permitted by law.


Indemnification

I agree to indemnify and hold harmless 4everBods Fitness Club from any loss, liability, damage, or cost incurred as a result of my participation in wellness services.


Agreement & Signature

I have read and fully understand this waiver. I acknowledge that I am signing it voluntarily and that it is binding.
